ANTI DRUGS SKATEBOARD COMPETITION

(1) This competition by AADK Sarawak is FREE entrance.
(2) Fill in your full details including the category you wish to join by ticking on the box provided. Please provide your real name as certificates of participation are provided.
(3) If you wish to join the Anti-Drugs kusus please tick on the box (E).
(4) The dateline for submission of forms is 20th October 2020 (today last day)
(5) Those below the age of 18 need to have parents permission to join by signing on the weaver form on page 2.
(6) Forms can be submitted online by inboxing or whatsapp to Mohammad Hamzah Shah (SKA) or Boy Hisham (AADK).
